WELCOME to Troop 574 and an exciting year ahead. Please take the time to register your son and yourself with the Troop for this exciting year of Scouting.
The following items are included in your registration packet. All forms except the Class 2/3 health forms must be completely filled out and returned with the appropriate fees to be registered. This information is due to the Troop before your son can participate in any troop activities other than regular Troop meetings.
Boy Scout Application - (Required)
Troop 574 Code of Conduct (2-up for duplex printing)
Troop 574 Code of Conduct (2-up for simplex printing)
Code of Conduct signature page. All Scouts and parents must sign.
Media Consent Form. Parents must sign this.
Class 1/2 Health Form – Medical and insurance information information (MUST include vaccination dates) (Scout & Registered Adult).
Class 3 Health Form - For adults 40 and over and Scouts attending high-adventure camps.
Adult Application - For adult leaders and adults that transport scouts and for Merit Badge counselor registration.
Merit Badge Counselor Information Form - For Merit Badge counselors (MUST include separate adult application).
Guide to Youth Protection Training - For Merit Badge Counselors and registered Adults to complete online training every 2 years.

Scouts Transferring from another unit (Cub Scout Pack or Boy Scout Troop) - $30
(already registered for current year)
New Scouts (not transferring) - $50
Adults Transferring from another unit (Cub Scout Pack or Boy Scout Troop) - $1
New Adults - $10
